First Visit

Greeting and Initial Consultation

In order to give you the best care possible, it is important that we get a comprehensive look at your medical history and properly assess your individual needs. This includes any current musculoskeletal history, as well as any past injuries, surgeries, accidents and more. We also take into consideration your lifestyle habits such as diet, sleep, exercise, and more. Below you will find our new patient form. Please take a few minutes to fill this prior to your first visit so we can spend more time discussing your case with you face to face. After the initial intake form, we will also take down any insurance information you may have so that we can verify your chiropractic benefits. Consultation

After completing your Intake Form, you will have an initial consultation with one of our doctors. At your initial visit, we will go over your health history, current issues, as well as address any questions you may have and your personal treatment goals.  During the consultation, please do not hesitate to ask any questions you have to the Doctor. The more understanding you have about your own healthcare, the better we can serve you and your needs. 

 


 

Examination and X-rays

After discussing your health history with you, the doctor will then perform a 10-15 minute examination that includes neurologic, orthopedic, postural, and physical tests. These tests help the doctor identify the root causes of your complaints. 

After the exam, if the doctor believes that xray imaging would be beneficial to further assess the state of your spine, you will receive x-rays in our office. The x-rays allow us to get a better picture of what is going on inside your body-we can see alignment, degeneration, as well as the health of your discs and bones. The doctor will typically go over the x-rays with you on your second visit, or report of findings. 

 


 

Report of Findings

With your health history, treatment goals, and examination, the doctor will explain what your examination results show and recommend a course of treatment for you. The report of findings will cover:

-Review of x-rays and exam findings

-Recommended treatment and care plan, including chiropractic care, passive therapy, and rehab and exercises. They may also suggest massage therapy and yoga classes.

 

 


 

 

Treatment

Our first goal at Aligned Modern Wellness is to first to get you out of pain as quickly as possible and back to doing the things you love. Our second goal is to help you strengthen your body and change any bad behaviors that may be the reason you landed here in the first place. Treatment plans are custom made for each patient and tailored to individual needs and schedules. We always listen to our patients questions and concerns when forming our treatment plan to get you back to feeling better as soon as possible. Dr. Treva and Dr. Kelley have several different methods that they utilize for adjusting their patients and will explain everything as they go through the treatment. 

 

Most treatment plans will include:

-Chiropractic Adjustment

-Soft Tissue therapies to ensure the muscles optimally support and hold the adjustment

-Physical rehabilitation to improve flexibility, strengthen muscles, and ultimately prevent recurrent flare ups.
